Health Declaration and Informed Consent

By confirming this booking, I declare that:

• The information I have provided is true and complete to the best of my knowledge.

• I will inform Lock and Flow Studio before participating if my health or medical circumstances change.

• I understand that Reformer Pilates is a physical activity and, like all exercise, carries an inherent risk of injury.

• I confirm that I will participate within my own capabilities, follow all instructor guidance, use the equipment safely, and immediately inform my instructor if I experience pain, dizziness or discomfort.

• I understand that instructors may modify exercises or refuse participation if they believe it would be unsafe for me to continue.

• I consent to Lock and Flow Studio seeking appropriate medical assistance on my behalf in the event of an emergency if I am unable to do so myself.

• I understand that this declaration does not affect my legal rights and that Lock and Flow Studio does not exclude or limit liability where this is prohibited by law.
